BEFORE PARTICIPATING
- I receive a letter asking me to take part in the project.
- I can find out about the project by consulting the project brochure or by visiting our website at www.cartagene.qc.ca .
- I am contacted by a recruiter to see if I wish to take part in this project.
- I may accept or not to schedule an appointment with a nurse.

THE MEETING WITH THE NURSE
- During the meeting, I may ask to the nurse any questions that I have about the project.
- If I want to participate, I must sign the consent form; my consent should be free and informed.
- The nurse will fill out a health questionnaire with me.
- The nurse will take some physical measurements: height, weight, muscle strength, lung capacity, bioimpedance (body fat percentage), arterial rigidity, bone density, partial electrocardiogram, hip and waist circumpference, etc.
- The nurse will also take a blood sample (about seven (7) table spoons) and a urine sample.

OPTIONS
- I have the option to fill out, at home, a genealogy questionnaire and a satisfaction questionnaire.
- I have the option to consent to be re-contacted for further studies linked to CARTaGENE.
